  <li><strong>Phone Number</strong> (required by the shipper)</li>
  <li><strong>E-mail</strong></li>
  <li><strong>Preferred Shipping Method:</strong>
    <ul>
      <li><strong>USPS:</strong> Often cheaper for small or lightweight items and may be faster if you are far from Arizona</li>
      <li><strong>UPS Ground:</strong> Typically more cost-effective for larger or heavier items; pricing is usually within a few dollars of USPS</li>
      <li><strong>UPS 3-Day Select:</strong> Available upon request for faster delivery but more costly</li>
